That's interesting. Perhaps it is the case that midi data controlled within the External Menu can be sent even when an internal Synth is active but data controlled within the Synth Section's External mode can't.

Can Mod Wheel, Ctrl Pedal, Volume Pedal and Swell pedal transmitted when an internal synth is active? I'm guessing these controllers can be switched on or off according to their Device MIDI A/B/C settings?

Pitch Bend probably can't be transmitted as like SusPed it is switched on/off in the Synth Section.

I have connected my NS4 to a Korg Kronos and want to do exactly what you describe under option 2: only send sustain pedal midi messages to the Kronos, not any note messages. In the NS3 the "non-keyboard" option in the EXTERN section allowed doing this, but on the NS4 I cannot seem to get this to work either. Have you been able to figure out how to make this work? If not, has anyone raised a request back to Nord to reinstate this non-keyboard feature from NS3 on the NS4?
